Emcara Health sat down with Healthcare Innovation to talk about delivering home-based care for vulnerable populations at scale. The interview includes a case study from our work with a large health system where we cut total cost of care by 27.5% while significantly reducing hospital utilization (10.7%) and ED visits (14.6%). It also highlights an innovative partnership in conjunction with providers across 17 counties in Florida to better serve Medicare members.
