Emcara Health’s multidisciplinary care teams may include primary care doctors, nurse practitioners, registered nurses, behavioral health specialists, pharmacists, nurse care managers, and more. On the frontline of these teams are community health workers who are hired from the communities they serve and are trained in an empathetic, empowerment-based approach. We build trusted relationships with patients, provide care wherever and whenever it’s required, and help those most in need live their healthiest lives.

Primary Care Physician (Doctor)

The primary care doctor leads your care team and oversees the care you receive.

Pharmacist

Your Pharmacist on your care team helps you with medications and ensuring you are able to get and take them as prescribed.

Behavioral Health Provider

Behavioral health providers on your care team can provide mental health care and substance abuse disorder care support.

Nurse Practitioner

Your nurse practitioner will visit you regularly, perform exams, order tests and labs, prescribe medication, and act as your primary provider.

Registered Nurse

Your RN will visits you and calls you in between visits with your RN to answer your questions and help you stick to your treatment plans and health goals.

Community Health Worker

Your community health worker helps you with all the non-medical things you need to stay healthy and stress-free – such as finding services you need or helping you understand your benefits.
