1) You may have received a letter or email informing you about annual health assessments at home from Emcara Health. We will call to schedule a visit at a time that works for you, or you can call us at 1-800-728-0901.
2) A nurse practitioner will come to your home to discuss your health and review your medications and existing treatment plans. You can also choose a video visit.
3) Emcara Health partners with your doctor and health plan by sending a summary of your visit to both! We may also provide education materials related to your specific health needs.
