Your in-home health checkup will be performed by Emcara Health, a leading provider of in-home healthcare and a trusted Martin’s Point partner.
Your annual in-home health checkup is a once-per-year visit like you get at your doctor’s office – but in the comfort and privacy of your home. A nurse practitioner will come to your home, conduct an exam, and review any medications.
You’ll have time to ask any questions about your health and will get advice tailored to your needs. During your visit, you’re welcome to have a friend or family member with you.
This personalized service is part of your Martin’s Point Generations Advantage plan and will be completed at a time that’s convenient for you, even on nights and weekends.
1) You may have received a letter or email informing you about annual in-home health checkups from Emcara Health. We will call to schedule a visit at a time that works for you, or you can call us at 1-800-691-7145. (TTY 1-800-967-1796).
2) A nurse practitioner will come to your home to discuss your health and review your medications and existing treatment plans. You can also choose a video visit.
3) Emcara Health will send a summary of your visit to your PCP, to help support the care they provide you.
